I use QTam.

I create my bitmap in CorelDraw and then I use it to convert to an Icon.

Its free and it works pretty good and its super simple.

There's a plugin for photoshop that allows you to "Save As" an icon.
It will convert any image type, preserve transparancy, etc.

The only thing you can't do is store multiple icons per file.

I tried to use the icon classes in .NET 2.0 but found they became quite
pixelated and lost a lot of definition. It could have been my
implementation, but most sources I found on the net all said that the
Icon conversion in .NET still leaves a lot to be desired.
I'm no expert, and just want nice icons on my forms and sys tray.
Photoshop plugin makes it easy.

I can't remember what its called though. It was on a page full of other
photoshop plugins. Its free. Google should assist you in this matter.

IconoMaker is no long free.  The download is trialware.

"Best" is subjective depending on your needs.

If you want a professional looking app, you should probably only consider 32 bit
versions either as XP style or png files, multi-res where required.  A plugin to
PhotoShop is handy (if you have PhotoShop), and, then there is the new so-called
Vista style icons that may be of future interest. 

I think most all the freeware icon maker apps deal only with the old style icons, 24
bit or less.
